   - Fix leak if nonce = 0 but reliability != none
   - More work on failure codes (ticket #788)
   - Fix race with _finished indication in OCMOSJ

     most did not have hashCode() implementations.
     These are never used as keys in a Set or Map or checked for equality.
     Fixes findbugs issues (ticket #379)
